APPOINTMENTS

All visits are scheduled by appointment only. The initial evaluation session is typically scheduled for up to 90 minutes.  Follow-up visits are scheduled to last between 25 to 50 minutes, depending on the treatment course we collaboratively decide to pursue. Out of courtesy for the next patient, I will end all sessions on time, even if you arrived late.  

CANCELLATIONS

All appointments made will solely be reserved for you. If you are unable to make a scheduled appointment, I request that you provide at least two business days (48 hours) notice or you will be charged the full fee for the missed visit.  Monday appointments need to be cancelled by 12PM the preceding Thursday.  Please note that insurances will not typically reimburse the fees for missed visits. 

PAYMENT POLICY

Fees are due upon completion of the visit unless other arrangements have been made in advance.  Please make checks payable to Lazaro Zayas, M.D.  and ensure that your name is visible on the check.  This office does not contract with any insurance carriers.  Your invoice statement, however, will include all of the information necessary to submit an insurance claim. 

COMMUNICATION

If you need to reach me in between appointments, please call my office phone at 617.299.9029.  All messages left are completely confidential and will be returned by me within one business day.  Because communication via email and text message is not secure and may compromise your privacy, I only communicate via email or text message to reschedule appointment times. Do not use email or text message in the event of any emergency. 

PRESCRIPTION REFILLS

Please allow two business days to obtain a prescription refill in the event you will run out of medication before our next scheduled appointment.  To obtain a refill, please call my office phone at 617.299.9029 and leave a message detailing your full name, birth date, medication name, dosage, frequency, and pharmacy telephone number. 

EMERGENCIES

In the event of an emergency, please call 617.299.9029 and I will try to return your phone call within one hour.  If the emergency is life-threatening and cannot wait, call 911 or proceed to your nearest emergency department. If I am away or otherwise unavailable, a colleague will be covering all calls. Do not use email or text message in the event of any emergency.
